#22: Turkish Airlines Flight 981 (1974)
Shortly after takeoff from Paris, a cargo door on the DC-10 failed. The sudden decompression ripped through the cabin floor, severing control cables. The plane went into a dive and crashed in a forest outside Paris. All 346 people onboard were killed in the worst crash of its time.

The investigation revealed a known design flaw in the cargo door latch. Warnings had been ignored, and the result was catastrophic. Afterward, cargo door designs were completely changed, and the disaster became one of the most infamous examples of corporate negligence in aviation history.
